Real Food 4 Kids Agreement *
Real Food 4 Kids agrees to offer cooking classes in exchange for payment by the client according to the following terms and conditions:
SAFETY: Culinary classes at Real Food 4 Kids are hands-on and students are required to use various kitchen utensils and cooking equipment. We are committed to safety and health at all times in the kitchen. Students are supervised at all times and will be given reasonably age-appropriate tasks. Parents should be aware, and hereby acknowledge, that participation at times involves using knives and other sharp utensils, and that children will be working with and in close proximity to kitchen equipment, including but not limited to hot surfaces and therefore Real Food 4 Kids cannot guarantee an injury free or risk free environment. Classes may also involve handling of raw food items, including meat. Safe sanitary practices will be reviewed with the students. By enrolling your child into Real Food 4 Kids culinary classes and by submitting this form, the parent/guardian hereby releases and covenants not to sue Real Food 4 Kids, its owners, members, agents, employees, representatives and assigns, from any and all liability for injuries and/or damages, regardless of cause or fault, incurred in connection with or arising out of attendance or participation in a Real Food 4 Kids cooking class or event.
SCHEDULING: Real Food 4 Kids reserves the right to cancel camps or classes and provide a full refund when camp or class minimum registration numbers are not met.
PAYMENT AND CANCELLATIONS: Class fees are non-refundable unless the class is cancelled. If you provide written notice 72 hours in advance that you are unable to attend, Real Food 4 Kids will issue a credit that you can use for a future class.
PHOTO, VIDEO OR OTHER RECORDING CONSENT: At the end of this form, you will be asked if you give consent to Real Food 4 Kids to use images or filmed footage of cooking classes in printed materials or on the web for the purposes of promoting Real Food 4 Kids classes. Please indicate if you do not wish to have your child photographed or filmed.
MEDICAL CONSENT: By signing this form the parent/guardian gives permission for medical treatment to be administered to their child by a qualified medical professional in the event of an emergency. Every effort will be made to contact the parent/guardian at the phone number listed above, but in the event that no one can be reached the signer authorizes Real Food 4 Kids to act on behalf of and in the best interest of the child.
STUDENT RESPONSIBILITIES: Students are expected to show respect for others, follow directions, and abide by all class rules. If students bring a phone or other device to class, Real Food 4 Kids requires the phone/device to be set aside during class. Students are welcome to photograph their culinary creations at the end of class. Real Food 4 Kids reserves the right to send a student home or decline their return if their behavior is inappropriate.
I acknowledge by checking this box that I have read, understand, and agree to be bound by all terms and conditions in this agreement, and that I have legal authority to enter into this agreement on behalf of the Student/Child to which this agreement applies.

Below are some helpful tips…
• If you have time to give your child a small snack prior to class, that would be great! If your child gets hungry before we're ready to eat our meal, I'll provide some fruit or veggies to munch on while we cook.
• Please use the side door located next to the garage for drop off and pick up, so that you enter into the kitchen.
• Please tie long hair back prior to class.
• Please remind your child that devices are not permitted to be used during class.
• Please be sure to advise me of any allergies in advance.
• We will be eating our creation towards the end of class. I'll send home any extras, along with recipes.
